Cry


The other day i drove my 3 year old daughter to her play school. somehow she was not in a good mood to go to school and she started crying the moment the car started rolling. To add to it , it started to rain. I tried to pacify her by getting some toffees on the way. The toffees were able to hold her tears back only for a while. The moment she got down the car, she started all over again. I could not forget her tearful face before she went inside the classroom. i couldnot work peacefully that day. My heart melted. Have you seen your child cry?

Dasavatharam


I saw the much hyped Dasavatharam yesterday. I got a free ticket for the night show. I didnot go with much interest. But watching the movie turned out to be an interesting experience. I have never appreciated that such a wonderfulful story teller resided in Kamalhasan. I have seen his previous movies where he had taken a major part in story and screenplay (eg. Virumaandi). Here he is simply terrific. The story telling is so smooth with small twist and turns (though you can guess them before hand), its like reading a Sidney Sheldon or Irving wallace or Dan Brown. You also have small seemingly insignificant jigsaw pieces which fall into the slot correctly at the end (eg. the significance of the scene where an entire local community is locked up in the mosque for inquiry). I thoroughly enjoyed the movie. The only black mark i could see was the overdone makeup for some characters. You feel them like puppets moving with real people. The best done part is that of Naidu. Hats off to kamalhasan for the efforts he has put on. He must reap the fruit.

case continued

I thought the ideal treatment would have been removal of the adenoma at an earlier stage itself or atleast now. 131 I ablation would be another choice. but in my practice i found that patients with toxic adenoma end up having hypothyroidism after RAI, though it should not happen theoretically. hence my choice would be a hemithyroidectomy in this case. In the given case medical therapy is being continued as per patient,s preference.

The patient brought her old records in the follow up visit. Ultrasonogram of the thyroid had shown a 5 cm nodule in the left lobe of the thyroid. Technitium scan done had shown a hot area corresponding to the nodule. a diagnosis of a toxic nodule is clear. what would be the ideal treatment in this case.

case history

I saw a patient today who is nearly touching her eighties. She was diagnosed to have thyrotoxicosis 14 years ago and had been on anti thyroid drugs for nearly 13 years before she decided to stop it 6 months ago. She went to a gastroenterologist 2 weeks back with increased stool frequency. She was referred to me by the GE after a complete GI evaluation. Examination was unremarkable except for an enlarged left lobe of the thyroid. Thyroid function test ws suggestive of a thyrotoxic state. I made a diagnosis of apathetic toxicosis and started her on antithyroid drugs again. I also advised her to bring her old records.
